2012 Medicare debate is all about the baby boomers (AP)
WASHINGTON ? Baby boomers take note: Medicare as your parents have known it is headed for big changes no matter who wins the White House in 2012. You may not like it, but you might have to accept it.
Dial down the partisan rhetoric and surprising similarities emerge from competing policy prescriptions by President Barack Obama and leading Republicans such as Wisconsin Rep. Paul Ryan.
Limit the overall growth of Medicare spending? It's in both approaches.
Squeeze more money from upper-income retirees and some in the middle-class? Ditto.
Raise the eligibility age? That too, if the deal is right.
With more than 1.5 million baby boomers a year signing up for Medicare, the program's future is one of the most important economic issues for anyone now 50 or older. Health care costs are the most unpredictable part of retirement, and Medicare remains an exceptional deal for retirees, who can reap benefits worth far more than the payroll taxes they paid in during their careers.
"People would like to have what they used to have. What they don't seem to understand is that it's already changed," said Gail Wilensky, a former Medicare administrator and adviser to Republicans. "Medicare as we have known it is not part of our future."
Two sets of numbers underscore that point.
First, Medicare's giant trust fund for inpatient care is projected to run out of money in 2024. At that point, the program will collect only enough payroll taxes to pay 90 percent of benefits.
Second, researchers estimate that 20 to 30 percent of the more than $500 billion that Medicare now spends annually is wasted on treatments and procedures of little or no benefit to patients.
Taken together, that means policymakers can't let Medicare keep running on autopilot and they'll look for cuts before any payroll tax increases.
Privatization is the biggest divide between Democrats and Republicans.
Currently about 75 percent of Medicare recipients are in the traditional government-run, fee-for-service program and 25 percent are in private insurance plans known as Medicare Advantage.
Ryan's original approach, part of a budget plan the House passed in the spring, would have put 100 percent of future retirees into private insurance. His latest plan, developed with Sen. Ron Wyden, D-Ore., would keep traditional Medicare as an option, competing with private plans.
Older people would get a fixed payment they could use for private health insurance or traditional Medicare. Proponents call it "premium support." To foes, it's a voucher.
Under both of Ryan's versions, people now 55 or older would not have to make any changes. GOP presidential candidates Mitt Romney and Newt Gingrich praise his latest plan.
How would it work? Would it save taxpayers money? Would it shift costs to retirees as Ryan's earlier plan did? Would Congress later phase out traditional Medicare? Those and other questions must still be answered.
"I'm not sure anybody has come up with a formula on this that makes people comfortable," said health economist Marilyn Moon, who formerly served as a trustee helping to oversee Medicare finances.
White House spokesman Jay Carney says the Wyden-Ryan plan "would end Medicare as we know it for millions of seniors," causing the traditional program to "wither on the vine."
But what administration officials don't say is that Obama's health care law already puts in place one of Ryan's main goals by limiting future increases in Medicare spending.
Ryan would do it with a fixed payment for health insurance, adjusted to allow some growth. In theory that compels consumers and medical providers to be more cost-conscious. Obama does it with a powerful board that can force Medicare cuts to service providers if costs rise beyond certain levels and Congress fails to act.
Like several elements of Obama's health care overhaul, the Independent Payment Advisory Board is in limbo for now, but it is on the books. If the board survives Republican repeal attempts, it could become one of the government's most important domestic agencies.
The White House wants to keep the existing structure of Medicare while "twisting the dials" to control spending, said a current Medicare trustee, economist Robert Reischauer of the Urban Institute think tank.
Ryan's latest approach is arguably an evolution of the current Medicare Advantage private insurance program, not a radical change, Reischauer said. That's particularly so if traditional Medicare remains an option.
"In the hot and heavy political debate we are in, participants are exaggerating the difference between the proposals," he said.
During failed budget negotiations with Republicans last summer, Obama indicated a willingness to make more major changes to Medicare, including gradually raising the age of eligibility to 67, increasing premiums for many beneficiaries, revamping co-payments and deductibles in ways that would raise costs for retirees, and cutting payments to drugmakers and other providers.
"I was surprised by how much the president was willing to offer in terms of Medicare changes without a more thorough vetting and discussion," said Moon. Obama says he will veto any plan to cut Medicare benefits without raising taxes on the wealthy.
Democrats are still hoping to use Ryan's privatization plans as a political weapon against Republicans in 2012, but the Medicare debate could cut both ways. For the 76 million baby boomers signing up over the next couple of decades, it will pay to be watching.

Nevada Welcomes First Domestic Solar-Geothermal Energy Facility
The Stillwater power station in Churchill County, about 80 miles east of Reno, will add a 24-MW array of solar photovoltaic panels (peak capacity) to an existing 27-MW geothermal power plant, making it the first of its kind in the U.S.
The newly combined facility is being built by Enel Green Power North America (EGP-NA), which is a division of Enel Green Power?the world?s largest renewable energy company.
EGP-NA owns and operates more than 70 plants with an installed capacity of around 800 MW in 20 states that are powered by renewable hydropower, wind, geothermal, solar, and biomass energy, and has some 500 MW of green power projects under construction in Nevada, Utah, Minnesota, Kansas, and Oklahoma.
The novel green-power project?which was announced in late August by Nevada Senator Harry Reid, Department of Energy Secretary Steven Chu, and EGP-NA President Francesco Venturini?will produce electricity that will be sold to local Nevada utility NV Energy.?
Joining green energy sources
When the solar array is completed, the 240-acre site will feature 7,000 poles supporting some 81,000 fixed polycrystalline-silicon photovoltaic panels that will convert the sun?s rays into power, said Bill Price, Vice President of Geothermal Construction and Engineering at EGP-NA. Meanwhile, Stillwater?s nearby hydrothermal wells will also continue to generate electricity.
The concept behind this variety of combined facility is to augment the current power source with solar energy during periods of high demand, such as during hot Nevada summers when air conditioning use spikes. Together, the complimentary systems can reliably supply the surrounding region with electricity 24 hours a day, seven days a week, Price said.
This groundbreaking installation would provide the nation with an opportunity to lead the world in this market.
At the introductory press presentation, Venturini reportedly said that he believes this kind of solar-geothermal plant will be popular among American utilities with access to geothermal resources.
?This is an incredible new technology that can be used across the country,? stated Secretary Chu, who added, ?All these things are going to be necessary as we move into renewables.? He noted that the groundbreaking installation would provide the nation with an ?opportunity to lead the world in this market.?
Binary geothermal plant
As it stands now, the Stillwater geothermal facility uses binary geothermal technology?the most common type of geothermal electricity plant being constructed today.
In its binary system, he explained, moderately hot (?medium enthalpy?) water from geothermal wells feeds into a heat exchanger where its heat passes to a secondary working fluid of isobutane, which has a much lower boiling point than water. Because the isobutane working fluid is heated in a closed-loop pipe system, the fluid flash vaporizes into pressurized gas. The high-pressure gas then drives a turbine-generator set, which produces electricity. The isobutane vapor is next condensed back into a fluid state by an air-cooled system.
The company is building Stillwater?s fixed solar array in-house using local expertise and labor.
EGP-NA?s Stillwater and nearby Salt Wells facility are the only geothermal plants in the world that use large-scale electric submersible pumps in the wells to pressurize and thus extract geothermal water. Combined vs. hybrid
The use of both solar and geothermal power-generation systems, Price noted, not only improves the power plant?s electrical production profile, but reduces costs and the facility?s parasitic load by allowing the combined system to make use of the same plant infrastructure?inverters, transformers, reactive power equipment, switchgear?that prepares the DC power feed for the AC electrical grid.
Although the combined PV solar-geothermal plant benefits from the co-location of the two renewable energy sources, it would be too much to describe it as a hybrid system, a term that implies a greater degree of integration.
Such a description would, however, apply to another type of solar-geothermal plant installation: a hybrid concentrating solar power (CSP)-geothermal facility. A true hybrid solar-geothermal power plant would be one in which a geothermal system and a CSP system would both heat the same working fluid that flows into the power-generation turbines. A spokesperson for EGP-NA said that it is in the early stages of developing such a ?test plant? in the same location in Nevada.

South Africa's Sun City to invest $800 mln in Sri Lanka tourism

COLOMBO (Reuters) - South Africa's Sun City resort will invest $800 million in a tourism project in Sri Lanka, the head of the island nation's state-run Tourism Board said on Friday.
Sun City resort is run by South African gaming and hotels group Sun International Ltd.
It will be the largest ever investment in the country's leisure sector, which is booming since the end of a 25-year war in May 2009.
"Sun City of South Africa is now going to make an $800 million investment in a 200-acre private property," Nalaka Godahewa, head of the Tourism Board told Reuters in an interview.
The investment will be made on Sri Lanka's hotel city in Katana, a coastal town located 15 km north of the commercial capital, Colombo, he said.
"That decision is finalized. That is one of the biggest investment."
Elanza Joubert, Sun City Resort's deputy inventory manager at events and entertainment department, said in an e-mail response to Reuters questions that the company would not comment on the plan until the new year.
The government said in July it was expecting at least $1.5 billion in foreign investment in a proposed "tourist city" that includes four five-star hotels, shopping and a convention centre in Katana. ? Continued...

For Gingrich, it's a struggle to stay on message (AP)
STORM LAKE, Iowa ? As he scrambles to stop a slide in Iowa, Newt Gingrich's strategy amounts to this: hammer home a message about jobs and the economy while wrapping himself in the mantle of Ronald Reagan. But the loquacious former House speaker keeps struggling to stay on message.
On a 22-stop bus tour of Iowa, Gingrich finds himself unloading on his GOP rivals and reviving talk of a Greek cruise that nearly sank his campaign earlier this year. He fields questions about his work for mortgage giant Freddie Mac, ethics allegations and whether his three marriages make him a polygamist.
The economy? Jobs? Those issues sometimes have been lost in the mix.
"It's been wild and woolly," Gingrich acknowledged to a voter as his wife, Callista, collected a double cappuccino at a Sioux City coffee shop.
If there was ever a time when Gingrich has needed the discipline he's long lacked, it's probably now, as polls show his support tumbling in Iowa in the wake of a storm of ads assailing him as a Washington insider who used his influence to line his pockets.
He now trails rivals Mitt Romney and Ron Paul in Iowa and even if he does manage to score in the top three in Tuesday's caucuses, he doesn't have the money or the organization at this point that those two opponents do as they prepare to go the distance in the state-by-state march to the GOP nomination.
Gingrich argues that his economic pitch is the key to victory, and he doubled down on it Thursday _or at least tried to.
He appeared in Storm Lake with noted Reagan economist Art Laffer, who praised Gingrich as "far and away the best person to bring this county back to prosperity." Gingrich outlined his tax-cutting economic proposal and implied he was the heir to Reagan's supply-side vision. But he also strayed into long-winded digressions on the federal government's regulation of particulate matter load and conflict in the Strait of Hormuz.
His trademark spray of ideas leaves some voters impressed ? but overwhelmed.
"He has so many," said Ruth Lawlor, 76, who came to hear Gingrich speak at a chocolate store in Algona this week. "It's hard to keep track."
Gingrich's predilection to go for the jugular also has tripped him up, earning his self-described "positive" campaign headlines that he didn't want.
In an interview on CNN this week, Gingrich took the bait.
He not only blasted Romney and Paul but used some of the most incendiary language of the campaign so far. Romney wasn't "man enough" to own up to the negative attacks launched at Gingrich. And Paul was "totally outside the mainstream of every decent American."
Just days later, Gingrich seemed to be suffering from selective amnesia.
"The strategy of focusing on jobs and economic growth, staying positive and being pretty relentless in answering questions at every meeting is working," he said Thursday.
At his campaign events, Gingrich encourages his audiences to fire away with questions about allegations made in attack ads.
In recent days, he's been asked about an ethics fine he paid as speaker and his work for Freddie Mac.
"I don't understand numbers with all those zeros," said a man in Thursday's crowd, referring to the $1.6 million Gingrich's company earned from Freddie Mac.
Gingrich explained that he didn't take in all that money himself and that he fought to increase regulations and not increase funding for the government-sponsored entity.
The candidate argues that such forums give him an opportunity to set the record straight on issues that have been distorted. But they also dredge up the controversies, even as he seeks to put them behind him.
One example came in a telephone town hall meeting Wednesday night when a caller likened Gingrich's three marriages to polygamy.
"Jesus very specifically states in the Bible that divorced people are really still married, which I think technically means now that you're a polygamist, and I'm wondering what you'll do to legalize polygamy in the U.S. if you were to be elected president," the man said.
Gingrich labeled the question "fairly unusual" and said he would oppose any effort to legalize polygamy.
The former Georgia congressman acknowledges his tendency to stray off script.
At Mabe's Pizza in Decorah he was asked why his Republican rivals have been so eager to embrace government intervention in the economy.
He paused and an impish smile crept across his face.
"I'll just get in trouble," he said.

N.Korea rallies for successor, no mention of nuclear arms (Reuters)
SEOUL (Reuters) ? North Korea in a policy-setting message for New Year called on its people to rally behind anointed successor Kim Jong-un by becoming "human shields" but made no mention of its nuclear arms program, the key source of regional security concern during his deceased father's reign.
The North's three main state newspapers said in "a joint editorial" published Sunday that Kim Jong-un has legitimacy to carry on the revolutionary battle initiated by his grandfather Kim Il-sung and developed by his father Kim Jong-il, who died two weeks ago of a heart attack.
"Kim Jong-un, the supreme leader of our Party and our people, is the banner of victory and glory of Songun Korea and the eternal center of its unity," the joint editorial carried by the North's state KCNA news agency said.
"The dear respected Kim Jong-un is precisely the great Kim Jong-il. The whole Party, the entire army and all the people should possess a firm conviction that they will become human bulwarks and human shields in defending Kim Jong-un unto death."
The joint editorial assailed the South Korean government for pursuing confrontation and war maneuvers despite efforts by the North to reopen dialogue, and it repeated its demand for the withdrawal of the U.S. military from the South.
But conspicuously absent from the 5,000-word New Year editorial was any mention of its nuclear arms program.
Momentum was building in diplomatic contacts between the North and the United States before the announcement of Kim Jong-il's death on December 19, raising expectations that the two sides may be closer to reaching a compromise to restart stalled talks aimed at ending the North's nuclear program.
Those talks stalled in 2008 when Pyongyang balked at intrusive inspections of its nuclear sites under a 2005 deal by six countries including the United States and South Korea to give the impoverished North aid in return for disarmament.
Last week, in a fiery message that marked the first communication with the outside world since Kim Jong-il's death, the North's National Defense Commission, which is seen as the apex of power, declared it would not deal with the current government in the South.
South Korean President Lee Myung-bak had angered Pyongyang by cutting off aid to the destitute neighbor when he took office in 2008 demanding nuclear disarmament and economic reform as preconditions to reopen food assistance and political engagement.
Tensions on the divided peninsula had reached a new peak in 2010 when the North launched an artillery barrage into a South Korean island killing civilians. The North was also blamed for a torpedo attack against a South Korean navy ship killing 46.
The North's state media said Saturday that Kim Jong-un has been officially appointed supreme commander of its 1.2-million strong military, two days after the official mourning for the dead leader ended in a move seen as a rush to solidify succession and boost the junior Kim's grip on power.
Kim Jong-un was named a four-star general and given the vice-chairmanship of the ruling party's Central Military Commission by his father in 2010.
Experts believe the untested new leader, who had only been groomed for rule since 2009, will rule with the aid of a close coterie that includes his uncle and key power-broker, Jang Song-thaek, at least in the early stages of the leadership transition.
