Tafamidis, approved for use in Europe and currently under review by the US Food and Drug Administration (FDA), is the first medication approved by a major regulatory agency to treat an amyloid disease, a class of conditions that include Alzheimer's.
Tafamidis treats a deadly nerve disease caused by transthyretin (TTR) amyloid fibril formation, or the accumulation of abnormal assemblies of the TTR protein. The drug inhibits TTR aggregation, and clinical trials have shown that it delays the typical progression of nerve destruction in polyneuropathy patients.
"The details in this new paper, combined with clinical trial data, show for the first time that an amyloid disease can be successfully treated by reducing the rate of amyloid formation," said Jeffery W. Kelly, chair of the Department of Molecular and Experimental Medicine, the Lita Annenberg Hazen Professor of Chemistry, and member of the Skaggs Institute for Chemical Biology at Scripps Research. Kelly is a senior author of the new paper, which appears in an advance, online Early Edition issue of Proceedings of the National Academy of Sciences on May 29, 2012.
An Array of Progressive Symptoms
While the naturally occurring or "wild type" transthyretin protein is prone to aggregate in older people causing cardiac disease, a variety of destabilizing mutations lead either to a primary cardiomyopathy or to early onset forms of polyneuropathy, known as TTR familial amyloid polyneuropathy, affecting about 10,000 people worldwide.
Familial amyloid polyneuropathy compromises the peripheral and autonomic nervous systems, with symptoms including sensory deprivation and pain, muscle weakness and wasting, and alternating constipation and diarrhea. In some familial amyloid polyneuropathy patients, cardiomyopathy can present later in the course of the disease.
In transthyretin amyloid diseases that present primarily as a cardiomyopathy, doctors have been able to stave off heart failure with a liver and heart transplant; familial amyloid polyneuropathy patients receiving a liver transplant can benefit, since the liver is the primary source of mutant, disease-associated TTR. For the 90 percent of patients surviving transplantation, this surgical form of gene therapy slows familial amyloid polyneuropathy progression, but does not stop it as the wild type transthyretin protein can continue to form amyloid.
Left untreated, the TTR amyloidoses are relentlessly progressive and inevitably fatal, with a course of about a decade from initial symptoms to death.
The Search for Treatments
Kelly began searching for TTR-amyloidogenesis-inhibitors in the mid 1990s, and a few years later began to focus on a family of TTR-binding compounds, the so-called benzoxazoles, whose basic design would further be elaborated into tafamidis using a structure-based drug design paradigm. In 2003, Kelly co-founded a Cambridge, Massachusetts-based biotechnology startup, FoldRx Pharmaceuticals (now a fully owned subsidiary of Pfizer), to develop these compounds and optimize one of them into an orally available drug for the treatment of the TTR amyloidoses. The result was tafamidis meglumine, whose preclinical tests remained unpublished until now.
Kelly and his colleagues designed tafamidis to bind to the natural, functional TTR structure (mutant and wild type), in a way that prevents it from deviating from this natural, functional form into the amyloid state. TTR's natural, functional form is a "tetramer" made from four copies of the protein. Amyloidosis occurs when these tetramers come apart and the individual TTR proteins ("monomers") undergo shape changes enabling them to misassemble into dysfunctional amyloid aggregates. Included in the TTR aggregate distribution are amyloid fibrilsprotein stacks made from millions of TTR monomersalthough researchers suspect that smaller, shorter-lived pre-amyloid aggregates do more direct damage to nerve cells and nerve fibers.
The early onset TTR amyloidoses are caused by inherited TTR mutations that weaken the tetramers' ability to stick together, producing monomers more likely to aggregate into amyloids and other aggregate structures. Fortunately, the TTR tetramer, which is the backup carrier of the thyroid hormone thyroxine through the bloodstream, has two unoccupied thyroxine-binding sites along its longest and weakest seam. Kelly and his colleagues designed tafamidis to grab either of these thyroxine-binding sites, in a way that bridges the seam and helps keep the tetramer from coming apart.
A Stabilizing Influence
The newly published molecular and structural data show that tafamidis does indeed stabilize TTR tetramers, under normal physiological conditions in the bloodstream and even under abnormal conditions when they would be much more likely to fall apart and reassemble as amyloids. Tafamidis has this stabilizing effect on tetramers of the normal "wild-type" TTR protein as well as on those made from disease-associated mutant and wild type TTR subunits.
"There are more than a hundred TTR mutations that cause amyloidosis, but the vast majority of those TTRs are capable of being bound by tafamidis and held in the natural tetramer state," said Kelly.
Throughout the development of tafamidis-type compounds, Kelly and his colleagues collaborated with the Scripps Research laboratory of Ian A. Wilson, who is Hansen Professor of Structural Biology and a member of the Skaggs Institute at Scripps Research. The Wilson laboratory specializes in the use of X-ray crystallography to determine the atomic structures of interacting proteins. Whenever a small molecule stabilizer of TTR was generated that afforded interesting biochemical stabilization, Wilson's team analyzed its structure. "By the end, we had determined more than 30 small molecule stabilizerTTR structures, in an effort to generate tafamidis and identify the molecular interactions that lead to stabilization of the natural TTR tetramer," said Stephen Connelly, a Wilson laboratory staff scientist who performed these structural studies and who was a co-author the paper.
The small molecule benzoxazole, tafamidis, that ultimately entered clinical trials was optimized for several criteria, including its ability to stabilize the TTR tetramer's weakest seam. "We found that one end of the tafamidis structure fits neatly into the tetramer's hydrophobic thyroxine-binding pocket, while at the other end it binds to nearby polar amino acids, both types of interactions bridging or stabilizing the two halves of the tetramer," Connelly said. The drug's stabilizing force greatly reduces the rate at which these tetramers come apart, and in so doing greatly reduces the rate of amyloid formation.
Tafamidis is considered an "orphan" drug because its initial intended treatment population is the relatively small polyneuropathy population. However, even wild-type TTR forms amyloid in 10 to 20 percent of the growing elderly populationleading to cardiomyopathy. Thus, this condition, and drugs such as tafamidis that can treat it, could be of growing interest to the pharmaceutical industry.

GARLAND, Texas -- High school students have raised $10,000?for the family of?a convenience store clerk, known as "Grandma" to?many in the Garland community, who was set on fire during a robbery attempt.
Nancy Harris, 76, died Friday night, nearly a week after the attack at the Fina gas station where she worked in South Garland.
On Saturday,?students from South Garland High School?returned to the place of the attack to hold a car wash and raise money for her family.

"I was just devastated. Its kind of outlandish that someone would go out of their way to do this,"?said Dylan Stooksberry, president of the school's student council. "To have that happen to someone who you know, to?a face that you know, to a friend, it's devastating."?
"It hurts deep down and honestly if someone is going to go out of their way to do something bad like that, I am going to go out of my way to do something nice," said Stooksberry.
The Fina gas station is just a block from their school. Many students, like DJ Valderrama, personally knew Harris.
"I came in the gas station and she showed me so much kindness by giving me a free drink,"?Valderrama said. "Whenever somebody shows you that kind of kindness if affects your life, really."

Despite the tragedy on Sunday at the Fina gas station, many students told NBC 5 that the incident has brought the community closer together.
Many?hoped this fundraiser will help honor Harris.
"I know Ms. Harris is looking down on us right now, I know she is happy," said Valderrama.
Matthew Johnson is charged with attempted capital murder in connection with this case. Those charges could be upgraded because Ms. Harris has died.

His war plan suggests possible outcomes such as ?Saudi Arabia threatened with starvation ? Islam reduced to cult status?.
A copy of the presentation was obtained and posted online by Wired.com?s Danger Room blog. The college did not respond to the Associated Press? requests for copies of the documents, but a Pentagon spokesman authenticated the documents. Dooley still works for the college, but is no longer teaching, said the joint chiefs of staff chairman, General Martin Dempsey. Dooley has refused to comment.

?It is therefore illogical? to continue with the current US strategy, which Dooley said presumes there is a way of finding common ground with Islamic religious leaders, without ?waging near total war?.
The course on Islam had been taught since 2004, but was not part of the required core curriculum. It was offered five times a year, with about 20 students each time.
Though Dooley has been teaching at the college since August 2010, it was unclear when he took on that particular class, called Perspectives on Islam and Islamic Radicalism.
The joint staff suspended the course after it had received a student complaint, and within days Dempsey ordered all service branches to review their training to ensure other courses do not use anti-Islamic material.
On Thursday, Dempsey said the material in the Norfolk course was counter to American ?appreciation for religious freedom and cultural awareness?.
?It was just totally objectionable, against our values, and it wasn?t academically sound,? Dempsey said. ?This wasn?t about ? pushing back on liberal thought; this was objectionable, academically irresponsible.?
He described his purpose as generating ?dynamic discussion and thought?, while noting that his ideas and proposals are not official US government policy and cannot be found in any current official defence department documents.
A Pentagon inquiry is seeking to determine whether someone above the professor?s level is supposed to approve course materials and whether that approval process was followed in this case, said Colonel Dave Lapan, a spokesman for Dempsey.

SANTO DOMINGO, Dominican Republic (AP) - Dominican relief pitcher Juan Carlos Oviedo says he has his U.S. visa, and will able to rejoin the Miami Marlins in August after serving a suspension from Major League Baseball.
Oviedo was caught playing under the fake name of Leo Nunez and spent the offseason in the Dominican Republic clearing up his immigration status. He said Friday that he had received a visa from the U.S. government and would be in South Florida on Monday.
His suspension will be for eight weeks, a person familiar with the case said. The person spoke on condition of anonymity because the suspension hasn't been announced.
Marlins reliever Edward Mujica, who spoke by phone with Oviedo, said his teammate is excited about rejoining the Marlins.
"He's happy, and I'm happy for him," Mujica said. "He had a bad secret and didn't say anything to anybody, and he's sad about that. Now it's good because everybody knows his real name. But it's going to be hard to call him J.C., you know?"
Oviedo, who turned professional in 2000, has said he used a fake name since he was young so he could say he was old enough to sign his first contract.
He played under the name Leo Nunez for seven major league seasons and had 36 saves and a 4.06 ERA in 68 games for the Marlins in 2011.
They acquired All-Star closer Heath Bell in the offseason, and Oviedo is expected to be given a setup role.

I am being charged for criminal damaging for keying a car. I did not key the car but someone i was with did. The guy's who's car was damaged told the police he thinks I did it, so they called me in for fingerprinting. I went and got prints because I knew they weren't mine and my original statement says I wasnt nowhere near his apartment. Well the next day the police said a cop ran my plates that night in his apartment complex and showed me the cop's statement which also stated I was with another female. So I told the truth about me not keying his car but the person I was with did. I am being charged and I go to court next week. How should I plea? What should I do?

You would expect that if that trend is for real, we should see a decline in the number of foreclosure-related notices being issued. Last month, that is exactly what happened.
In April, fewer than 190,000 of the notices were reported. That makes it the lowest monthly total in 5 years (and a decline of 5% from March). In other words, although it takes some time for a foreclosure to occur, the writing seems to be on the wall: the high water mark in foreclosures may well have been passed.
Another sign: the average price of completed foreclosures rose from the year?s average of $226,953 to $256,027. Lower supply, higher prices ? if the early trend continues,local foreclosure bargain hunters may soon find themselves having to hunt a bit harder.
There are many online resources that provide foreclosure lists: properties that are in good shape as well as distressed foreclosure properties, pre-foreclosure properties, REO foreclosures and foreclosure auctions. Anyone thinking about taking advantage of the bargains that are still out there (and they ARE still out there) should consider consulting an experienced Evansville agent to help with the process.

Lindsey Rosman, a doctoral student in the department of psychology at East Carolina University contacted me recently about a confidential online survey she is conducting as part of her research into body image and sexual functioning after breast cancer?.
Lindsey is also a breast cancer survivor, so knows first hand how often quality of life concerns associated with cancer survivorship can be overlooked. In her email to me she writes:
From my perspective as a survivor and as a researcher, these important issues are poorly understood, leaving many women without adequate support and treatment. With support from my faculty supervisor and approval from the Institutional Review Board at East Carolina University, I designed a confidential online survey to examine?body image concerns and long-term sexual adjustment in women diagnosed and treated for early-stage breast cancer. ?
I believe that for most women, their body, and their breasts in particular, are a symbol of what it means to be a woman, mother, and sexual partner. Therefore the impact of breast cancer and its treatment is not limited to changes in physical appearance, but also can include alterations in how survivors perceive themselves as women and partners in an intimate relationship.
By conducting this research, my goal is to learn about women?s body image concerns and sexual experiences in the context of cancer survivorship and ultimately develop an effective intervention to improve the quality of life for survivors and their loved ones.

'Everyone said I was going to have a boy, and they were right!' Snooki tells In Touch magazine. By Gil Kaufman

She was hoping for a brood of mini-Snooki's running around under-heel in her home with fiancé Jionni LaValle, but "Jersey Shore" star Nicole "Snooki" Polizzi is overjoyed that her bouncing bundle will wear blue. "Everyone said I was going to have a boy, and they were right!" Snooki told In Touch magazine in the new issue. The baby is due in September and the couple are narrowing down name choices, with the two leading contenders at the moment being Lorenzo or Jionni Jr.
Snooki, 24, told the magazine that she's very excited about the birth of her baby boy, but she admitted that she was hoping for a Jersey girl. "I thought it was going to be a girl. I was hoping it would be, because all girls want girls," she said about her initial disappointment. "It's still my baby, no matter what. I'm excited either way!"
Snooki displays her growing bump on the cover the current issue of the supermarket tabloid, sharing that she's switched from her go-to craving for pickles to cold treats. "I love anything cold and juicy, like Italian ices," she said. In addition to her notoriously rabid sex drive cooling off for now, Snooki said she's also ready to leave some other childish things behind.
"I'm not going to party hard anymore, even after the baby is born," Snooki said. "Being a mom changes you."
Since the rumors first popped up earlier this year, Snooki has flashed the engagement ring she received from LaValle, who has appeared on "Jersey Shore" in the past. Snooki has been filming her spin-off with "Shore" bestie JWoww and an MTV spokesperson said in March that whatever shenanigans the two are up to have been going quite well. "Nicole and Jenni began filming last week and moved into their new digs with animal print furniture in tow ... Things are going great thus far."
Snooki and LaValle began dating a year and a half ago. According to the MTV star, he has plans to pursue a career in education as a teacher. He also works with kids as a wrestling coach.
